Claire House aims to offer a service to families of children and young people from 0 - 23 years of age living in Merseyside, North Wales, Cheshire and the Isle of Man who have a life threatening or life shortening medical condition.  

Referrals can be made by family members or any professionals involved with the child/young person.  This would generally be when the child/young person is under 16 years of age.  However, it is recognised that there may be exceptions to this and all referrals are looked at on an individual basis.  Written consent is then obtained from parents/guardians for permission to request medical details from the child/young person's GP/Hospital Consultant, or any other appropriate professional.  In most cases, a home assessment visit will take place to further add to this information although, in some instances where the medical condition has a clearer prognosis, this may not be necessary.  The Admission Panel will then consider all the available information and decide whether or not Claire House is the most appropriate environment to meet the individual needs of the child/young person.  The family and all key professionals will be given written confirmation of the outcome of the referral.  

Every child/young person who is accepted at Claire House will then be reviewed on an annual basis to assess the appropriateness of Hospice care provision.  Special consideration would be given as to how best to meet the needs of young people in transition.
